Let's be blunt: an Orc Druid is a 4/10 synergy build because you are fighting an uphill battle against your own armor class. With an optimal starting DEX of 12 and the Druid's restriction against metal armor, you'll be sitting at a miserable 13 AC in Hide armor and a shield. You are going to get hit. A lot. However, the Orc's racial traits provide unique, highly specific band-aids for the Druid's fragile controller chassis.
Adrenaline Rush is your bread and butter here. As a bonus action, you can Dash and gain temporary hit points equal to your proficiency bonus. When a melee brute gets in your face while you're concentrating on Sleet Storm, you don't have to waste your action Disengaging. You Dash away as a bonus action, soak the potential opportunity attack with your new temporary HP, and use your action to cast Thorn Whip or Dodge. This completely changes how you position on the battlefield.
Then there is Relentless Endurance. Dropping to 0 hit points instantly breaks your concentration on encounter-ending spells like Conjure Animals or Wall of Fire. Build Path (Levels 1-20)
Levels 1-4
Start with the Circle of Stars subclass at level 2. Because your 12 DEX makes your AC abysmal, you need a way to protect your concentration. Starry Form: Dragon ensures you cannot roll lower than a 10 on Constitution saving throws to maintain concentration. Combine this with your 14 CON, and you'll auto-pass DC 10 checks. Levels 5-10

Cantrips

Level 1-2 Spells
Entangle and Spike Growth are mandatory. Spike Growth creates a 20-foot radius of hazardous terrain. If an enemy gets through it and reaches you, use Adrenaline Rush to Dash backward, forcing them to walk through the spikes again to catch you.
Level 3-5 Spells
Conjure Animals, Sleet Storm, and Wall of Stone. Sleet Storm shuts down enemy spellcasters by forcing concentration checks and knocking them prone. Take this at level 8. Bumping your CON to 15 and adding your proficiency bonus to concentration saves is mathematically superior to War Caster by Tier 3, especially since Starry Form: Dragon already creates a high floor for your rolls.
Tough
Take this at level 19. By this tier, enemies deal massive chunks of damage. Adding 40 hit points to your pool keeps you comfortably above the threshold where massive damage rules might instantly kill you, bypassing Relentless Endurance.
Gear Progression
Tier 1 (Levels 1-4)
Equip a Wooden Shield and the best non-metal medium armor your DM allows (usually Hide armor, putting you at 13 AC with 12 DEX). Keep a Yew Wand as your spellcasting focus so you have a free hand for somatic components.
Tier 2 (Levels 5-10)

These heavy frontliners can lock down the enemies you trap inside your Spike Growth or Entangle. Since your 8 STR and 12 DEX make you a liability in melee, you need them to act as a meat shield.
You also synergize perfectly with a Phantom Rogue. Your Faerie Fire gives them reliable Sneak Attack without needing to hide, and your Orc Darkvision means neither of you needs to carry a torch, allowing you to execute flawless ambushes in subterranean dungeons.
Multiclass Options
Cleric 1 (Peace Domain)
A one-level dip into Peace Cleric is incredibly potent. Common Pitfalls
- Wasting Adrenaline Rush early: Temporary HP does not stack. Do not spam your bonus action Dash on turn one if you aren't actually moving anywhere useful, just to get the temp HP. Save it for when you need to escape.
- Misunderstanding Relentless Endurance in Wild Shape: If you drop to 0 HP while in a Wild Shape (like a badger for scouting), you revert to your normal form and take the carry-over damage. Relentless Endurance only triggers if that carry-over damage drops your Orc HP pool to 0.
- Using Symbiotic Entity: Do not play a Circle of Spores Orc Druid. The temporary HP from Symbiotic Entity is completely overwritten by the temporary HP from Adrenaline Rush, ruining the subclass's main feature.
- Trying to be a melee combatant: With 8 STR and 12 DEX, your melee DPR is a pathetic 3.9 at level 5. Never use Shillelagh. Stay in the backline and cast spells.
